Contract Adoption Contract Adoption Contract Russell Rescue of Tennessee Adoption Agreement. I hereby acknowledge receiving the animal described in this agreement and agree to take full responsibility for the animal. I agree to provide the animal with a safe, clean and comfortable living environment and to provide the animal with adequate food, water, and attention at all times. I agree this animal will be primarily an inside pet, and WILL NOT BE LEFT OUTSIDE UNATTENDED AT ANY TIME. I agree to take the animal to a veterinarian within 2 weeks of adoption for an initial checkup and also annually for yearly tests and vaccinations. I agree to keep the animal on heartworm preventative medication at all times and to provide any and all necessary veterinary care for the animal throughout its life. I understand that starting from the date of this adoption contract, I am responsible for all medical care for the animal, unless any exceptions are noted on this contract by an authorized representative of Russell Rescue, Inc. I agree to license the animal in compliance with the laws and ordinances in the municipality where I reside, and I will obtain rabies vaccinations as required by law and keep them up to date. I attest that if there are any other animals in my home, they are all current on their annual vaccinations. I fully and completely release Russell Rescue, Inc. from any claim, cause of action or liability for any illness my other animals may develop, even if the said illness may have been contracted from the animal adopted from Russell Rescue, Inc. I understand that the adopted animal has been in a shelter environment and has been exposed to illness. While all reasonable care has been taken to assure the animals health, the animal may still potentially carry an illness such as kennel cough, works or other parasites. I agree to keep a collar with identification on the animal at all times. I AGREE THAT IF AT ANY TIME IF I DECIDE THAT I CAN NO LONGER KEEP THE ANIMAL, I WILL CONTACT RUSSELL RESCUE, INC. AND WILL RETURN THE ANIMAL TO RUSSELL RESCUE INC. I WILL NOT RELINQUISH THE ANIMAL TO ANY OTHER PERSON OR SHELTER OTHER THAN RUSSELL RESCUE, INC. In the event that I do return the animal to Russell Rescue, Inc., for any reason, I will at no time assert any claim against Russell Rescue, Inc. for any charges that may have occurred in connection with the animal. As Russell Rescue, Inc. is a non-profit rescue organization, all adoption fees are considered to be a donation and are non-refundable. if at any time the animal must be returned to Russell Rescue, Inc., I agree to forfeit all monies tendered to Russell Rescue, Inc. To help Russell Rescue, Inc. insure the future welfare of the animal, I agree to cooperate with any follow-up calls or visits from an authorized Russell Rescue, Inc. representative. I agree that this animal will be kept solely as a companion animal, and will not allow the animal to be used for any other purpose. I understand that Russell Rescue, Inc. cannot guarantee the health, temperament or training of the animal listed on this contract and hereby release Russell Rescue, Inc. from all liability once the animal is transferred to my possession. I hereby accept the possession of, title to (subject to the terms of the contract), and responsibility for the animal described on the contract and hereby release and discharge Russell Rescue, Inc. from liability for any injury or damages to any person or any person’s property caused in the future by said animal, and from any cause of action, claims, suits or demands whatsoever that may arise as a result of such injury or damages. I agree to be governed by the laws of Tennessee in all matters relating to this animal and release, hold harmless, and agree to indemnify Russell Rescue, Inc. from any liability for injury or damage caused by this animal. I hereby bind myself to the fulfillment of the above clauses and realize that failure to comply with these clauses will result in the repossession of the animal and possible legal action. I attest the information provided to Russell Rescue, Inc is truthful and accurate to the best of my knowledge. It is agreed that Russell Rescue, Inc. retains superior title in the animal listed in this contract for the express purpose of assuring the animal’s well-being. Russell Rescue, Inc. will only exercise its superior claim in the event that, in the judgement of Russell Rescue, Inc., proper and humane care as specified in the above contract provisions is not being afforded to the said animal. In such case, the animal may be taken through a claim and delivery proceeding. In addition, I agree to pay Russell Rescue, Inc. the sum of $500. 00 as liquidated damages in the event the terms of this contract are breached. This liquidated damage value is agreed to for the purpose of establishing value of the animal and does not bar Russell Rescue, Inc. from seeking return of the animal by judicial process or other legal means. I agree to pay all attorney fees and court costs in the event this matter if forwarded to any attorney for enforcement. In the event that it is discovered that the dog(s) that I am adopting are harmed though my negligence, or my failure to abide by the terms and conditions set forth in this agreement, I understand that I will be subjected to vigorous prosecution under the appropriate penal code in Tennessee or any other state and will be assessed ALL court costs pertaining to the recovery of this dog and any subsequent veterinary expenses incurred. I further understand that any expenses incurred by Russell Rescue, Inc. to reclaim said dog will be the responsibility of the adopter. I have paid the agreed adoption fee of $300.00 (three hundred dollars) per dog which can be paid by credit card, personal check or cash. Note: If the adoption fee is paid through PayPal, then an $8.00 non-refundable processing fee will be added. Russell Rescue of Tennessee is a non-profit 501c3. Please do not ask us to reduce our adoption fee. By doing this you are actually asking us to reduce the chances of us saving more lives. Also, please consider giving an additional one-time or ongoing donation in the form of money, food, blankets, crates, pads, leashes, etc. We incur thousands of dollars in vet bills every month and the rescue cannot survive on adoption fees alone.
